Please reach out to your CSM to confirm once you have completed the required attestation. If you have not completed and confirmed completion of all required attestations by May 11, you will be unable to run Medicare checks.
Before you start
You will need the following information:
- Name of the authorized signer for your Medicare enrollment
- Primary email address (and optional secondary email)
- Your PTAN (Provider Transaction Access Number)
- Your NPI
- The Contractor Code associated with your Medicare enrollment
- The Trading Partner Unique ID: 274B
Note: This attestation must be completed by the provider or an authorized representative. Clearinghouses or billing services should not complete this form on behalf of the provider.
Step 1: Authorization
- Open the HETS EDI Enrollment form.
- Review and accept the Terms and Conditions.
- Indicate whether your organization is located outside the United States.
- Enter the Authorized Signer Information:
- First and last name of the authorized signer (as enrolled in PECOS)
- Primary email address
- Secondary email address (optional)
- Enter your PTAN and NPI.
- Select the Contractor Code associated with your PTAN/NPI.
- Select Continue.
Step 2: Attestation
Trading Partner Information
- In the Unique ID field, enter the Trading Partner Unique ID 274B
- Select Confirm.
- Verify that the Trading Partner name appears and that a match is found.
If the name does not match, stop and confirm the Unique ID before continuing.
Provider Information
- Confirm that your PTAN, NPI, and Contractor Code have populated correctly.
- Select Validate.
Effective and End Dates
- If no prior relationship exists, the Effective Date will default to today.
- You may adjust the Effective Date if needed.
- Leave the End Date blank (it will default to 12/31/9999).
Do not select “Delete Existing Relationship” unless you are intentionally removing authorization.
Add Entry
- Select Add to include this attestation entry.
- You may add up to 10 NPIs/PTANs in a single submission if applicable.
- Once all entries are added, proceed to confirmation.
Step 3: Confirmation
- A confirmation message will display once the attestation has been successfully submitted.
- You may exit the form or start a new attestation if additional NPIs need to be added.
